INSTRUCTIONS

You can complete and submit your registration online by scrolling to the end of these instructions or click on Choice B to print a paper copy.
Once registration form is submitted, please send e-transfer to tickets.cornwallconference@gmail.com to confirm your registration to the conference OR
Send payment by cheque payable to Cornwall Fall Conference which can be mailed or given directly to Huguette, including cash, by calling her at the phone number or mailing address below. If you wish, once you complete the form, you can also scan it at cornwallconferenceaa@gmail.com

The banquet dinner seating is arranged to accommodate eight guests per table. Please note that specific seating arrangements cannot be guaranteed.

Names to be printed on ID tag for Banquet (Your group name can be added)

Select # of tickets for registration only at $20 each (no banquet)

Names to be printed on ID tag for registration only (Your home group name can be added)

Please provide names from your list of attendees with dietary restrictions and include the specific restrictions for each person listed.
